How to register

To register come into the surgery and we will ask you to complete some forms:

• A health questionnaire to help us to look after your needs as fully as possible.
• A registration form

Both forms can be downloaded from the above "Registering Online" tab.

We will also ask for evidence of identity and of residence in our area - a current utility bill plus photo ID would be ideal.

You will be allocated a Doctor and you will have the right to express a preference to see your allocated Doctor.  However, if this is not possible, you will be offered an appointment to see another healthcare professional.

If you are already taking medication, or have medical needs of any kind, we will invite you for an early appointment with either the doctor or nurse.

Medical treatment is available from the date of registration. Please contact reception for further information.

Change of Personal Details

If you change your name, address or telephone number please give full details to reception.  If you move outside our practice area, you will need to find a doctor in your new area.  The Practitioner and Patient Services Agency will be able to help you. Telephone: 01962 853361

Online 'Pre-Registration' With The Practice

If you wish to pre-register click on the links below to open the forms. When you have completed the 3 forms, please click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the forms to confirm that the details are correct.

Please note that you will not be registered  with the practice until you have signed the registration form in person at the surgery.

New Patient Registration Form

Child Registration form - for Patients under 16 years of age.  This form can be downloaded and emailed to the Surgery.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

New Patient Registration Form

The practice uses Access to Communication, a local interpreting and translation service which can provide face to face and telephone interpreting in five core Asian languages and most European and Middle-Eastern languages.  Please inform the reception staff in advance if you require this service.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
