Community Nurses

Our team of community nurses offer an excellent service to adult patients in the community who are unable to access practice based services. We have a very good working relationship with this team, and work together with them in a large number of very different situations, often doing joint visits to ensure the best care for our patients.

We meet with the Community Team on a monthly basis to share clinical updates and focus on the patients needing their clinical input to improve their health and well-being.

Health Visitors

A Health Visitor is a Registered Nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
