How to register
If you wish to register please come into the surgery and a receptionist will be happy to provide you with registration forms and a health questionnaire. We will require two forms of identification – photo ID and proof of address.
Once you have completed both forms an appointment with a practice nurse for a health check will be offered.
This provides us with basic health information about yourself and previous medical history before we receive your full medical records.
You will be allocated a Doctor and you will have the right to express a preference to see your allocated Doctor. However, if this is not possible, you will be offered an appointment to see another healthcare professional.
Change of Personal Details
If you change your name, address or telephone number please give full details to reception. If you move outside our practice area, you will need to find a doctor in your new area. The Practitioner and Patient Services Agency will be able to help you. Telephone: 01962 853361
